Terms and conditions for the provision of health care and related services

1. Introductory provisions

1.1 These Terms and Conditions regulate the rights and obligations between the providers of rehabilitation services and the clients (hereinafter referred to as the “client”).

1.2 The providers of services are Mgr. Jiří Hájek, ID: 22122745 , and Mgr. Zdislava Tučková, ID No.: 22122664 , Bc. Pekařská 635/6, 158 00 Praha 5 -Jinonice (hereinafter referred to as the “Provider”).

2. Services provided

2.1 The Provider offers individual physiotherapy, individual and group exercises, individual trainings and massages (hereinafter referred to as the “Services”).

3. Reservation of services and deposits for services provided

3. 1 The services are provided on the basis of a prior telephone, e-mail or personal reservation or on the basis of a reservation via the Rehatab reservation form, see section 12 of these Terms and Conditions (contact details).

4. Price and payment terms

4.1 The prices for the services provided are indicated on the provider’s website or in the price list available at the premises.

4.2 The provider reserves the right to change the price list without prior notice. The Provider may not change the prices of services already ordered. The new price list for the services provided will always be published on the provider’s website and in the price list available at the premises.

4.3 Payment for services is due immediately after each service is provided. Unless otherwise agreed

4.4 Payment can be made in cash, by bank transfer or by QR code at the premises.

5 Cancellation and cancellation policy

5.1 The customer is obliged to attend the agreed date at the place of business or cancel or change the reservation of this date no later than 24 hours before the booked date. This can be done either in the booking system or by contacting the contacts listed in clause 12 of these terms and conditions.

5.2 The Provider reserves the right to cancel the appointment. In the event that the Provider has to cancel the booking, the Customer will be offered an alternative date.

5.3 In the event of cancellation or rescheduling less than 24 hours prior to the scheduled date, the Provider reserves the right to charge a cancellation fee of 50% of the price of the booked service.

5.4 If the Customer fails to attend the booked service without prior notice, a cancellation fee of 100% of the price of the booked service will be charged.

6. Service Liability Guarantee

6. 1 The Provider is obliged to provide the services with professional care and in accordance with the applicable legislation, including laws and regulations relating to the provision of health services.

6.2 In the event that the services are not provided in accordance with the contract and these terms and conditions, the Customer has the right to make a claim in accordance with clause 7 of these terms and conditions.

6.3 The Provider has arranged health service provider liability insurance in accordance with the applicable legislation.

6.4 The Customer shall notify the Provider of any deficiencies in the services provided without undue delay.

6.5 The Providers shall not be liable for any damage caused by the Customer’s unprofessional conduct or failure to comply with the instructions of the therapist.

6.6 The Customer is obliged to inform the therapist about his/her health condition, possible contraindications and other relevant facts that may affect the course and results of the therapy.

7. Complaints Procedure

7.1 The Customer has the right to complain about the services if they are not provided in accordance with the concluded contract or these Terms and Conditions.

7.2 The complaint must be submitted without undue delay after the discovery of the deficiencies, in writing or in person at the provider’s premises, or by e-mail to the following address: “enterfyzio@gmail.com”.

7.3 The provider undertakes to resolve the complaint within 30 days of its receipt.

8. Privacy Policy

8. 1 The Provider processes the personal data of customers in accordance with the applicable legislation on the protection of personal data.

8.2 Personal data is processed for the purpose of providing services, issuing documents and communicating with the customer.

8.3 The customer has the right to access, correct or delete his/her personal data or limit the processing.

9 Ownership of the website design and logo

9.1 The website design and logo used by the Providers are protected by intellectual property rights and are the exclusive property of the Providers.

9.2 Any unauthorised use, reproduction, distribution or other disposal of the website design, logo or other elements protected by the Providers‘ intellectual property rights without the prior written consent of the Providers is prohibited.

9.3 All rights to the website design and logo, including copyright, trademarks and other similar rights, remain exclusively with the providers.

10. Governing law

10.1 These terms and conditions and the contracts concluded between the providers and the customers are governed by the laws of the Czech Republic. All disputes arising under these terms and conditions or contracts shall be settled under the law of the Czech Republic by the competent courts.

12. Final Provisions

12.1 These Terms and Conditions are valid and effective from 2. April 2025.

12.2 The Provider reserves the right to modify the Terms and Conditions at any time. The Customer will be informed of any changes to the Terms and Conditions in an appropriate manner.
